What the inspector wrote

On Thursday, August 7, 2025, Licensing Program Analyst (LPA) Hanna Cha met with licensee Loreto Ayala. The purpose of the inspection was to conduct an unannounced case management inspection to follow-up on two Type A citations that were issued on 7/2/2025 regarding licensee being absent from facility for more than 20% of operating hours and for having an uncleared adult providing care and supervision. LPA Cha disclosed the purpose of the inspection to the licensee. Upon arrival, there was one infant and one preschool aged child in care with licensee and licensee\342\200\231s assistant providing care and supervision. The current days and hours of operation are Mondays through Fridays, 7:00am-6:00pm. During the visit, licensee stated that the uncleared adult is no longer employed at facility. Licensee also stated that Child #2 (uncleared adult\342\200\231s biological child) who was present during the inspection conducted on 7/2/25, is not enrolled in facility. Licensee will provide both written statements to LPA Cha via email by 8/11/2025. LPA Cha completed a safety inspection of the facility at approximately 9:00am. No safety concerns observed at this time. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted and the report was reviewed with the licensee Loreto Ayala.
