What the inspector wrote

On 10/15/2024, Licensing Program Analyst (LPA) Lisa Clayton conducted an unannounced Required Inspection at Catalyst Kids \342\200\223 South Bay. LPA Clayton met with Site Supervisor Rita Brenes. LPA Clayton observed 18 children being supervised and cared for appropriately by 6 fingerprint cleared staff. Facility hours of operation are Monday \342\200\223 Friday, 7:00am \342\200\223 6:00pm. The CCC provides breakfast, lunch, afternoon snack, water and milk. LPA Clayton toured the facility indoors and outdoors for a Health and Safety Inspection. Capacity and limitations as specified on the license are being maintained. All children are under visual supervision of a teacher at all times. The CCC is in compliance with Title 22 regulations regarding Staff to Student ratios. There are no pools, ponds, Jacuzzis, spas or any other bodies of water on the property. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked cabinet area. LPA Clayton observed fully charged 2A:10B:C Fire Extinguishers in the following areas: \302\267 The front lobby \302\267 Classroom 4 \302\267 Classroom 6 \302\267 Classroom 7 \302\267 The hallway (outside of bathroom #2) LPA Clayton observed and tested carbon monoxide/smoke detector combos as well as a carbon monoxide detectors in the following areas: \302\267 Classroom 4 \302\267 Classroom 6 \302\267 Classroom 7 LPA Clayton inspected napping equipment placement, bedding and storage for safety. Water temperature, toilet paper, paper towels, safety and sanitation were inspected. Floors in the facility are clean and safe. LPA Clayton inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All hand washing facilities are in safe and sanitary operating condition. LPA Clayton observed the urinal in bathroom #2 to fill up with water after flushing and the isolated toilet in the same bathroom to be leaking. LPA Clayton advised Site Supervisor to update the department on the status of repairs to bathroom #2. LPA Clayton conducted a Health and Safety inspection of the food storage area, and all kitchen storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. LPA Clayton reminded Site Director Rita and Cristina (cook) that opened foods are to be labeled with the date it was opened. The kitchen (located upstairs) has a fully charged 2-A:K Fire Extinguisher and the pantry (located on the 1 st floor) has a fully charged 2:A10B:C Fire Extinguisher. LPA Clayton reviewed 5 children\342\200\231s files and observed files to have current contact information of parents and authorized representatives, and authorization for medical treatment, signed Parent\342\200\231s Rights and signed Personal Rights receipts. LPA Clayton reviewed 6 employee files which were found to be in compliance with immunization for childcare providers and Mandated Reporter Training, and CPR/First Aid as required. Site Supervisor Rita was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Incidental Medical Services \342\200\223 IMS are currently being provided for 3 students. LPA Clayton observed and inspected the areas where the medication is kept and reviewed the Medication Action plan for all 3 students. For IMS information see PIN 22-02-CCP . Site Supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. LPA Clayton reminded the staff to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, no Deficiencies were cited. Site Supervisor Rita will forward copies of the Medication Action Plans to LPA Clayton and the department via email. During the exit interview the report was reviewed and a copy was provided to Site Supervisor Rita. LPA Clayton posted a Notice of Site Visit which must remain posted for 30 days.
