Follow-up on corrections·January 21, 2026

On 1/21/26, Licensing Program Analyst (LPA) Jeanine Lipsey conducted a Case Management - Deficiencies visit. LPA met with Director Maureen Perera. LPA toured the facility and observed 33 children in care supervised by 5 adults. Based on Guardian record review, Staff Monique Baker, present today, date of hire of 10/15/25, was not associated to the facility until 1/21/26. Based on Guardian record review, Leticia Castro, date of hire 4/22/24, was not associated to the facility until today 1/21/26. Although this person was associated today, her finger prints are not yet cleared. An Type A and civil penalties was issued. LPA Jeanine Lipsey informed Director Maureen Perera that this report dated 1/21/26 documents One Type A citation which shall be  posted for 30 consecutive days as there is immediate risk to the health, Safety, or personal rights of children in care. Also, LPA Jeanine Lipsey informed the licensee to provide a copy of this licensing report dated 1/21/26 that documents any Type A citation to parents/guardian of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report.  A signed Acknowledgement of Receipt of Licensing Report (LIC9924), or written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the Director Maureen Perera. A notice of site visit was given and must remain posted for 30 days.
