What the inspector wrote

On 1/21/26 at 10AM, Licensing Program Analyst (LPA) Jeanine Lipsey a conducted an unannounced, continued in-person Case management incident inspection.  The incident was reported to Community Care Licensing on 1/9/26 via phone.  The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident is on going. Incident: Two of the children have behavioral issues. Upon arrival, LPA was guided on a tour of the facility by Director Maureen Perera to whom the purpose of the inspection was announced. Census:  There were thirty-three children being supervised by five staff. During the inspection, LPA obtained a second chance letter and conducted interviews with 5 staff. This incident needs further information. Exit interview conducted and report was reviewed with Director Director Maureen Perera  A Notice of Site Visit was given and must remain posted for 30 days.
