What the inspector wrote

On 3/10/26 at 8:33 AM, Licensing Program Analyst (LPA) Jeanine Lipsey a conducted an unannounced, continued in-person Case management incident inspection. The incident was reported to Community Care Licensing on 1/9/26 via phone.  The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident is on going. LPA met with Assistant Director Brittany Wijesekera. Director Maureen Perera arrived at 9:30am. LPA arrived when all the children were on the grounds singing the morning song. There were twenty-nine children being supervised by four staff. Incident: Two of the children have behavioral issues. During the inspection, LPA obtained a second chance letter, the facility roster, employee roster, Parent handbook, termination letter and conducted interviews with 5 staff. The incident was reported to the department because there are children that were diagnosed with behavioral issues. Per staff the children are hard to handle without assistance. The parents have been spoken to regarding their behaviors and the center requested they get a behavioral therapist to assist. Per staff after finally having a verbal meeting and the parents agreed upon a plan to help both parties, a second chance letter was given and the children are doing better. During the investigation LPA discovered that another child was terminated for their behaviors. This child is not diagnosed with a disorder; per the parent the child is just a typical 3-year-old and does not have issues at home. Per staff, the child would make noise while the other children where napping, the child would climb tables and call the teachers names. No reports or incidents were reported to the department regarding their behaviors. No reports documented to the parents were provided. Per staff the incidents were posted on Brightwheel to the parents as they occurred. The Parent handbook discipline policy, that was given to the parent at enrollment, states “if challenging behaviors persist or escalate, a parent conference will be scheduled to discuss a contributing factor and to develop a collaborative behavior plan”. Per parent, a conference was never scheduled, nor did it happen, to discuss the child’s behavior and or plan. The child was given a termination letter on January 20, 2026. The center did not supply any evidence of a conference or plan before the child was te rminated . Exit interview conducted and report was reviewed with Director Maureen Perera A Notice of Site Visit was given and must remain posted for 30 days.
