What the inspector wrote

Per Reporting Party, the Kidsville USA Child Care Center (CCC) sign-in and sign out logs are often incomplete and Subsidy Program timesheets are completely filled out which do not correlate with Kidsville USA CCC sign-in and sign out logs. Per Director, the facility uses an App called Bright Wheel where each parent has a unique identification code for parents to sign in and sign out their children daily, however, the Subsidy Program timesheets are separate, and not all parents sign them since those pertain to subsidy programs. The Director confirmed that the Bright Wheel App does not capture signatures only the sign in and sign out time. When interviewing staff, Staff #1, Staff #2, Staff #3 and Staff #4 disclosed that they have daily sign in and out logs, however, the Licensee/ Director and Assistant Director oversee the timesheets that are for children who receive subsidy payments. Per Parent #3, Parent #4, and Parent #5 disclosed that they sign in and sign out their children via the Bright Wheel App daily but there is no legal or digital signature just a check mark. Due to Director, staff, and parents disclosing that children are accounted for daily via the Bright Wheel App that does not capture a daily signature, the facility is not in compliance with 101229.1(a)(1) The person who signs the child in/out shall use their full legal signature and shall record the time of the day. However, when it comes to Subsidy Program timesheets that pertain to subsidy payments, those are documents that are separate from daily sign in and sign out written procedure that the Department requests. Based on LPA observations and interviews which were conducted and record review, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 Chapter 3, Section number 101229.1(a) (1) Sign In and Sign Out and a plan of correction is due 10/10/2025, a Type B Citation is being cited on the attached deficiency page. The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Maureen Perera, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. **Page 2 of 2
