What the inspector wrote

On April 30, 2026, at approximately 2:00 PM, Licensing Program Manager (LPM) Emiko Bell and Licensing Program Analyst (LPA) Janet Gil conducted an unannounced annual inspection. LPA/LPM met with Assistant Director Tara Perera and explained the purpose of the inspection. The School Age License operates Monday through Friday from 2:00 PM to 6:00 PM with an early arrival on Tuesday from 1:20 PM to 6:00 PM. At the time of inspection, there were 23 school age children present with four staff members, including the director. The facility was operating within its licensed capacity and maintaining staff-to-child ratios at the time of inspection. LPA/LPM inspected indoor and outdoor child care areas for health and safety hazards. The School Age Program operates in three classrooms: Classroom 9, 10, and 11. Classrooms were observed to be clean and equipped with age-appropriate materials, and furniture. Children’s personal belongings were stored in cubbies within each classroom. The facility is equipped with a fire alarm system installed throughout the building. Carbon Monoxide Alarms were observed in each School Age Classroom. LPA tested alarm in Classroom 9 and 10 which were observed to be working. LPA observed a fully stocked and accessible first aid kit. Multiple fire extinguishers were observed on site. Poisons, cleaning solutions, and other hazardous materials are stored in inaccessible area, in locked lower shelves. The facility provides food services for supper. Menus were observed to be properly posted inside each classroom. Drinking water is available to children indoors and outdoors with individual water bottles and water dispensers. Continue on Page 2... Continued, Page 2... The outdoor play areas are fully enclosed and equipped with a built in play structure with padding underneath. No pools, spas, or other bodies of water were observed in the day care areas. Children are signed in and out through Bright Wheel App system, by the front entrance of the facility. The facility’s license documentation and required postings were properly displayed and available for review inside the main office. The school age program conducts monthly emergency drills. The last documented disaster drill was conducted on April 2, 2026. LPA reviewed 6 children’s files and 3 staff files. All children’s and staff files were complete. Multiple present staff do not have Pediatric CPR and First Aid certification. A Type B citation is being issued on this day in accordance with the California Code of Regulations, Title 22. Based on record review, the licensee did not comply with the section cited above in having a valid Pediatric First Aid/CPR certificate for 4 out of the 5 staff that provide transpiration which poses a potential health, safety or personal rights risk to persons in care. Staff files also contained proof of required immunization, which were made available for review . All present staff have Mandated Reporter Certification on file.Facility was informed that as of September 1, 2016, a person may not be employed or volunteer at a child care facility unless he or she has been immunized against influenza, pertussis, and measles or qualifies for an exemption pursuant to Health and Safety code 1596.7995 and 1597.662. The director is aware that all staff are required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com . LPA observed the completed certificate on file. LPA encouraged the director to frequently visit our website at www.ccld.ca.gov for licensing regulations and new updates. The director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Continue on Page 3… Continued, Page 3... Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPAs verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP. A Type B citation is being issued on this day in accordance with the California Code of Regulations, Title 22. Please refer to LIC 809D. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Assistant director, Tara Perera .
