What the inspector wrote

On 01/14/2026, Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced case management inspection at the facility. LPA met with facility representative and stated the purpose of today's visit. The purpose of the visit is to obtain information and documents related to a change of director. On 11/20/2025, the facility representative expressed to LPA J. Culbert that they are the new director and has been since 07/27/2025. LPA J. Culbert provided them with a new director checklist and requested that all necessary documents be sent in to the Department to update the facility's information. Today , LPA inquired about the documents related to the director change. During today's visit facility representative emailed LPA the following documents: -Designations of facility responsibility LIC 308 -TB clearance -Teaching credential -Statement of Acknowledging Child Abuse (LIC 9108) Per facility representative, they will continue to gather all necessary documents. LPA informed facility representative that they will return at a later date. A notice of site visit was given and must remain post for 30 days. Exit interview was conducted and report was reviewed with facility representative.

This pose a potential health, safety, or personal rights risk to person in care.
