On March 20th, 2024, Licensing Program Analyst (LPA) Alemoh conducted an unannounced case management inspection to follow up on an Unusual Incident that happened on 03/15/2024. Child #1 was left unattended in the play yard for under a minute. Another classroom came outside right after C1 classroom and observed C1 sitting in the play yard area. Facility notified parents. Teacher was placed on suspension. LPA toured the facility and observed the play yard area where the incident occurred along a with a total of 116 children with 18 staff on the premises as well as both directors. LPA conducted interviews and obtained other supportive documentation related to the unusual incident. Based on the information gathered and interviews conducted, the facility made adequate corrections and adjustments to the incident that occurred. Parents provided the facility a sincere statement letter regarding the incident that occurred. T1 was provided with more training, along with all staff will be provided with more training on name to face protocols and procedures. No citations or deficiencies will be issued on this visit. However, during the time of this visit LPA Alemoh and Director Jackie discussed training's methods that will be conducted for all staff such as: Name to face protocols and procedures for supervision and safety. An exit interview was conducted and a copy of this report was provided to the licensee, along with her appeal rights and Notice of Site Visit.
