What the inspector wrote

On May 17, 2024, Licensing Program Analyst (LPA) Alemoh conducted an unannounced case management inspection to follow up on an Unusual Incident that happened on 05/10/2024. Child #1 was playing hot potato with Child #2 when C2 threw the toy back to C1 and hit him below his left eye and cheek and caused a bruise. An ice pack was provided for C1. Two staff members were supervising and observed the incident. Parents were notified. LPA toured the facility and observed the play yard area where the incident occurred along a with a total of 109 children along with 18 staff members and the Director on site. LPA conducted interviews and obtained other supportive documentation related to the unusual incident. Based on interviews conducted with staff and record review, S1/S2 provided first aid to C1 by providing an ice pack along with proper care. Upon further video review it was determined that the children were playing hot potato with a stuffed animal when C2 was observed throwing the stuffed animal to C1. C1 did have his hands up waiting for C2 to throw the stuff animal to C1. When C2 threw the toy back at C1 the toy appeared to have went through C1 hands causing the toy to hit C1 in the face below the eye. C1 was observed walking towards to S1 for aid and help where S1 was observed checking on C1 eye and providing an ice pack, cleaning and care. S1 notified the Directors of the incident, to which Directors contacted the parents. C1 did not go to the hospital, and later returned to the facility on Monday 13th of May. The facility handled the situation accordingly by informing the directors and parents of the incident, along with providing First Aid to C1. The facility conducts on going training's every month for all staff such as: policies and procedures training on children injuries and ways to prevent them. No citations will be issued today. An exit interview was conducted and a copy of this report was provided to the licensee, along with her appeal rights and Notice of Site Visit.
