At 8AM on 9/24/25, Licensing Program Analyst (LPA) Angela Luz arrived to the facility and met with Staff 1 (S1) Jeanne Burley. S1 informed Director is due to arrive at 8:30AM. LPA toured the facility with S1 and observed 8 preschool aged children with one staff. 8:30AM Director arrived and LPA discussed the purpose of the visit to conduct a Case Management - Other visit. At 10:27AM, LPA observed 29 preschool aged children with four staff. LPA informed that on report dated 12/16/24, the facility was cited for one Type B deficiency:1596.7995(a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Staff files did not contain documentation that staff were immunized against pertussis and measles. LPA conducted a file review of all staff files today, 9/24/25, and confirmed that all staff have documentation of immunization against pertussis and measles. A Letter of Deficiency Citations Cleared is issued today, 9/24/25. No deficiencies are issued today. A notice of site visit was given to Director Martina Beltran and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the Director Martina Beltran.
