What the inspector wrote

On 06/12/2025, Licensing Program Analyst (LPA) Tyra Chavies conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 05/30/2025. Upon arrival, LPA met with Director, Susie Taylor.\302\240 LPA informed director about the purpose of the visit and toured the facility. LPA observed 22 children being supervised by 8 teacher. Incident Detail: RP states... T1 observed C1 lean over and drop hand to the ground while on the back of a small bike in motion. C1 hand was ran over by the bike's back wheel which resulted in a hand injury. T1 reported the incident to the director. S1 and T2 called parents. Parents arrived about 20 minutes later. Child has returned back to the facility. On 06/12/2025 LPA Chavies observed children in care, conducted interviews with director and C1, received personnel and medical documents. Based on interviews conducted and review of\302\240 documentation, facility did follow the California Code of Regulations (CCR) and the Health and Safety policies and procedures. There are no deficiencies being cited. An exit interview was conducted with Director, Susie Taylor, this report was read and a copy issued. Notice of site visit was provided and must be posted for 30 days.
