An Informal Office Meeting was conducted on June 13, 2025, at 10:47 AM at the Palmdale Regional Office (PRO). Present at the meeting were Licensing Program Manager (LPM) Claretta Yates, Licensing Program Analyst (LPA) Crystal Ali, Licensee Yvonne Cottage, Matthew Mazas (employee), and Brooklyn Alcott (union representative). The purpose of the meeting was to address the licensee’s interest and future involvement with the Family Child Care Home (FCCH) and to review recent and past concerns regarding the operation of the facility. An agenda was prepared and discussed in detail. The agenda that was provided to the licensee on June 10, 2025 has been updated to conclude all past and recent concerns regarding the operation of the facility. The objective of this meeting was to support the licensee in achieving and maintaining compliance with the California Code of Regulations, Title 22, in alignment with the Department’s mission. H & S 1597.622 Employee Immunization During the meeting, LPM Yates discussed a recent application received noting the licensed address as the intended property for the licensee. LPM discussed the license shall not be transferred to other individuals or locations, it is imperative that licensee notifies parents and turns in her license if she is choosing to close her license with Community Care Licensing to allow for another individual to apply. Licensee plans to submit an updated family child care home application (LIC 279) to include Matthew Mazas as a licensee. Licensee informed LPM that she agrees that she will relinquish her position as licensee if she is not available in the home 80% of the daily operating hours. At the conclusion of the meeting, Licensee Yvonne Cottage acknowledged her understanding of the responsibilities of the Community Care Licensing Division, as well as her role and obligations as a licensee.
