What the inspector wrote

On 09/17/2024 Licensing Program Analyst (LPAs) Tyra Chavies and Ranita Richmond, conducted an unannounced 3 year Annual Inspection at Mary Star of the Sea Preschool. LPAs met with Director, (Elizabeth Castagnola) and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday 7:30 AM to 4:00 PM. LPA Chavies confirmed the phone number is: (310) 707-7873. There were 19 children in care being supervised by 3 Staff members. AM/PM snacks are provided by the center and parent are required to provide their child's lunch.\302\240 There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises.\302\240 Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. Indoor furniture and equipment are in good condition, free of sharp, loose and/or pointed parts. The facility floors, bathroom and classrooms meet Title 22 Health and Safety regulations. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPAs inspected and verified that there is a fire system connected throughout the facility. The facility does have a fire extinguisher which has an inspection date of June 2024 on the premises. Facility has one or more functioning carbon monoxide detectors. Drinking water is available both indoors and outdoors. Areas around climbing equipment have cushioning material to absorb falls.\302\240 All materials and surfaces accessible to children are toxic free. The facility is free of flies and insects. CHILDREN'S RECORDS: LPA Chavies reviewed 10 children's files. All files are up to date and are in good condition STAFF'S RECORDS: LPA Chavies reviewed 5 staff files. All staff records are update and in good condition. The following were also discussed with the licensing: The licensee is to provide to parents/guardians accepting services and to new parents/guardians the following for 12 months: A copy of any licensing reports that document a Type A citation. The licensee shall post citation for 30 days and during daycare hours. Individuals who are 18 years of age or older working in center must obtain a criminal record clearance.\302\240 Individuals within one month of their 18th birthday must be fingerprinted immediately . Failure to obtain a criminal record background check clearances prior to initial presence will result in an immediate $500.00 dollar civil penalty and $100.00 more per day until licensee is in compliance Commencing September 1 st , 2016, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. New Appeal Process : A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. New Immunization Requirement: Law enacted by SB 277, beginning January 1 st , 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into childcare or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint. When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN). Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, there are no deficiencies being cited. An exit interview was conducted with Director, Elizabeth Castagnola. A copy of this report was read and given to Director as well as LIC 9213 (Notice of Site Visit form.) LIC 9213 is required to be posted for 30 days.
