On Thursday, August 1, 2024, at 11:45 am Licensing Program Analyst (LPA) Mayra Rivera conducted an unannounced Case Management incident inspection to follow up on an Incident Report dated 7/25/24. During this visit, LPA Rivera interviewed director and 2 staff. Based on the interviews there is no evidence of personal rights violation and reporting requirements were met. No Title 22 or Health and Safety violations occurred; therefore, no deficiencies were given during this in visit. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report and appeal rights were reviewed with the director Aura Osoy.
