What the inspector wrote

On Friday, May 22, 2026, Licensing Program Analyst (LPA) Evelyn Garcia conducted an unannounced case management visit to follow up on an Unusual Incident Report concerning an incident that occurred on March 24, 2026. It was reported to the Department on March 24, 2026. LPA met with Site Supervisor, Aura Osoy. LPA observed 38 children under the care and supervision of 8 staff members. The incident report indicates that on 3/24/26 the facility was advised from a child’s parent, that the child had bruises on his arms and the child stated it was done by a teacher. During this inspection, LPA Garcia conducted interview with site supervisor, education coordinator and obtained documentation related to the case management. Furthermore, LPA also completed a safety inspection of the center, no deficiencies were observed. Teacher files were not reviewed due to staff files being kept at the main administrative office not on this location. Further investigation is required in order to resolve the case management. An exit interview was conducted, a copy of this report, and notice of site visit were provided to Aura Osoy, Site Supervisor.
