What Happened

Case management inspection for lead action level exceedance. Water testing from 10/21/2022 showed 30.4 ppb at outdoor Outlet A (exceeds 5.5 ppb action level). Facility failed to post results within 24 hours and did not notify parents of exceedance or remediation plan, resulting in Type B violation.

Observations

  • Lead water testing on 10/21/2022 showed Outlet A with 30.4 ppb (exceeds 5.5 ppb Action Level Exceedance)
  • Outlet A located outdoors outside of Room 35
  • Point of use filter installation not verified despite facility claiming installation on 02/27/2023
  • Lead testing results not posted within 24 hours of receiving results from Department
  • Parents not notified of lead exceedance results or remediation plan
  • No indoor water outlet for children to drink from - children use reusable water bottles and donated bottled water
