, Licensing Program Analysts (LPAs) Katrina Chicote and Franchesca White conducted an Unannounced Required \342\200\223 1 Year inspection to the above facility. Upon arrival, LPAs disclosed the purpose of the inspection and initially met with Adult 1(A1) who allowed entry to facility and provided tour. A1 states she lives in the home and assists with providing food to the children. LPAs observed A1 does not have clearance at time of inspection. This is a violation of Title 22 regulation requirements and poses an immediate health, safety, and personal rights risk to children in care and a civil penalty assessed. Licensee, Chanta Deeminter, arrived at a later time
and joined the inspection. Adults in the home were discussed. There was
initially one
infant present during today\342\200\231s inspection. Licensee states that there are currently
seven
children enrolled. The children's roster was reviewed and is current. Per Licensee, the facility\342\200\231s hours of operation are Monday thru Friday 12:30 PM to 3:00 AM
. LPAs discussed Overnight Care procedures with Licensee and license limitations that care for children is limited to 23 hours. Emergency Disaster Plan, License, and Parents\342\200\231 Rights were posted at the time of inspection. Disaster drill log was also available during today\342\200\231s inspection, last disaster drill conducted on

home which consists of
three
Bedrooms,
three
Bathrooms, Kitchen, Living room, with a back house as Day Care Activity Space that includes Bathroom 3. Main Care Areas used by the children include the back house as Day Care Activity Space which is located in the rear of property. LPA discussed entrance procedures to Day Care Activity Space with Licensee at time of inspection. Per Licensee, areas off limits to children and parents include
entire front house which includes all three Bedrooms, two Bathrooms, Kitchen, and Living Room. Per Licensee, front home is used for the isolation of ill children, LPAs observed standard blue cots leaning on wall in Living Room at time of inspection. LPAs observed that there is a large iron gate blocking access to front home making it off limits to children in care.
Beginning at 1:35 PM, all areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. LPAs observed portable A/C unit in Day Care Activity space. There is a working telephone maintained in the home. LPAs observed child size tables and chairs, shelves filled with large legos, balls, foam blocks, puzzles, art

materials, and age appropriate toys, free of loose and sharp parts available for children. LPAs observed Day Care Activity Space with Kitchenette, per Licensee she provides food to children in care and children eat in Day Care Activity Space.
At 1:42 PM, LPAs observed Bathroom 3 to have operable toilet and sink, adequate hand washing and toileting supplies, and to have general area safety and sanitation at time of inspection. LPAs observed under sink cabinet to be used for storage for additional toileting supplies. LPAs discussed storage of detergents, cleaning compounds, medications, and other items which can pose a danger need to be inaccessible. LPAs observed some of these items stored on high shelf in Kitchen in off limits house. The Licensee does understand that poison must be locked with a key or combination lock.
Per Licensee, the children will have access to rear area of backyard for outdoor play. LPAs observed this area is accessible through front door in Day Care Activity Space. At 1:44 PM, LPAs observed outdoor play area has grass and adequate perimeter fencing through-out the yard.
LPAs observed bicycles, basketball court, and other age appropriate toys available for the children in the backyard. At 1:47 PM, LPAs observed grassy area that Licensee states children use to have large holes. LPAs discussed this observation with Licensee at time of inspection. LPAs observed two large dogs stored in side of the house that were blocked off preventing access to rest of the home at time of inspection. LPAs did observe dogs jumping and barking during inspection. Per Licensee, dogs are kept there when children are present at the facility.
At 2:08 PM, LPAs observed the valve on the required 2A 10BC fire extinguisher indicates fully charged with no service tag or proof of purchase receipt at time of inspection. At 1:42 PM, LPAs observed Ssmoke and carbon monoxide detectors are in operable condition. LPAs observed first Aid kit is kept in the
Day Care Activity space

The Licensee has current Pediatric First Aid and CPR. Proof of immuniz

against influenza
, pertussis, and measles was not readily available during today\342\200\231s inspection
. The Licensee did not have proof of completion for AB1207 Mandated Reporter training at time of inspection.

Per Licensee, there are no weapons, firearms in the home, LPAs observation did not observe any firearms and/or weapons in the home. At 2:00 PM, LPAs observed an above ground jacuzzi in the front area of the backyard which is an off limits area. LPAs observed front area of home to have gate above five feet that has a self latching door making front area where above ground jacuzzi is located off limits to children in care.

Licensee (or facility representative) was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility.
The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

AB 1207
: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.
Volunteers are encouraged but not required to take the training.

Senate Bill 792
:
this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.
Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health & safety.
Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type \342\200\234A\342\200\235 deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).
The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child\342\200\231s file immediately upon receipt from parent.
Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.
