What the inspector wrote

On 01/21/2025 at 1:39p.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection due to an incident that occurred on 09/27/2024 and was reported to the Regional Office. LPA met with Tamar Odabashian, Center Director and informed the nature of the visit. At the time of the visit there was only 34 children and 13 teachers for Headstart. According to the UIR on 09/27/2024 at 2:10p.m. Center Director Tamar Odabashian was informed of an incident that occurred on 09/26/2024 that involved a child and teacher during outdoor/transition time. ( S1) was providing water to the children. ( C1) was attempting to bite another child and teacher put her hand on (C1) forehead and pushed child's head back so that (C1) would not bite the other child. The incident was observed by two teachers who were outdoors with another classroom. The two teachers were from a different center locations on that day to provide support. The day that the incident occurred the Center Director Tamar Odabashian was absent. LPA Whitmore was unable to obtain a copy of the incident report that was given to mom LPA Whitmore reviewed ( C1) file. There was no incident report in file. The documents that were obtained were the Zine Incident & Fact Finding Inquiry. During the investigation LPA Whitmore interviewed ( S1) and ( C1). Based on the information obtained further investigation is needed. Copy of report, Appeal Rights, and Notice of Site Visit was issued to Center Director Tamar Odasbashian.
