On February 22, 2023, at 9:45 am Licensing Program Analyst (LPA) Evelyn Garcia conducted an announced case management visit to follow up on an Unusual Incident Report concerning an incident that occurred on January 24, 2024. It was reported to the Department on January 25, 2024. LPA Garcia met with Joe Villareal who gave a tour of the facility. At this facility there are 4 classrooms. Upon arrival there were 56 children under the care and supervision of 9 teachers and 4 administrators. The incident report indicates that on 1/24/24 the child's mom called after picking up the child and stated she observed a mark on the child's forehead, staff stated there were no marking's when mom picked up at the site and the mom came to school the next day and threatened to close down the school. During the inspection today, LPA Garcia conducted interviews with staff members and 3 children, reviewed children's files, viewed the facilities video recording of the when the child was leaving the facility on date of the incident. The teachers personnel files were not reviewed due to files being kept at the administrative office at 12502 Van Nuys Blvd, Suite 220, Pacoima CA 91331. A visit will be scheduled to review staff files. Further follow up is required at this time. An exit interview was conducted, a copy of this report and notice of site visit were provided to Erika Mendoza, site supervisor.
