What the inspector wrote

On January 6, 2025, Licensing Program Analyst (LPA) Portia Bowden conducted an unannounced Annual Required Inspection at the above facility. LPA met with Site Supervisor Natasha Stallworth, explained the reason for visit , provided LIC 125 (Entrance Checklist) and was guided on a tour. Per Site Supervisor the facility operating hours are Monday through Friday from 7:30AM\342\200\223 5:00 PM. LPA observed all required documents on the facility licensing board (PUB 269, PUB 393, LIC 610, LIC 9224, LIC 613A). This is a preschool program servicing ages 2 to 5, consisting of 2 open classrooms, and an outdoor play area. Facility also houses on the premises an infant program with a toddler component under facility number 197416986. At 12PM, LPA observed 20 children supervised by 4 staff in the preschool Classroom. Capacity and limitations as specified on the license are being maintained. Per Site Supervisor there are 32 children currently enrolled. The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Landline Telephone service is maintained by the facility. LPA observed Central air throughout facility with proper ventilation. At 12:15PM LPA observed an operable Carbon monoxide detector as well as a smoke detector in the preschool Classroom. At 12:17PM LPA observed a 2A10BC Fire Extinguisher fully charged with a service tag of 12/24 on the office wall. LPA observed sufficient amount of cots for sleeping, Per Site Supervisor Linens are washed by the facility once a week or as needed. LPA observed 2 Age appropriate sinks and 2 age appropriate potties in good repair in the classroom. Per Site Supervisor Potty training is provided. At 12:20PM LPA observed a medication box mounted on the office wall, locked an inaccessible to children in care. LPA observed all medication inside to be properly labeled with children\342\200\231s name. Per PIN 21-21-CCP lead testing to be conducted for all equipment used for drinking consumption. Per record review, LPA observed negative lead test results conduct on 6/24 LPA observed Disinfectants (bleach and water solution concocted weekly by facility) stored in a high cabinet inaccessible to children in care. Per Site Supervisor there are no poisons on the premises. Per Site Supervisor facility provides Breakfast, Lunch and a PM Snack. All food is supplied by the facilities main Central kitchen and brought here pre packaged. Per Site Supervisor there are children in care with food allergies. At 12:20PM LPA observed a food allergy list on the wall of the classroom. Per Site Supervisor Central Kitchen has the food allergy list and sends alternatives food options. Per Site Supervisor Outside food is not permitted except for children with food allergies. Parents are permitted to send food or beverage alternatives if a Doctor\342\200\231s note is on file. All food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard. LPA advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility. LPA Reviewed 5 staff files, and observed them to be complete. LPA observed Teacher\342\200\231s Assistant Stephanie Jones Pediatric CPR/ First Aid with an expiration date of 12/25. All staff had Current Mandated Reporter Training. LPA reviewed 5 children\342\200\231s files and observed them to be complete. The name of the child care center director or fully-qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. No Deficiencies were observed during today\342\200\231s inspection. Exit interview conducted and report was reviewed with Site Supervisor Natasha Stallworth. No deficiencies were cited during inspection. A notice of site visit was given and must remain posted for 30 days
