What the inspector wrote

Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection due to a self-reported incident that occurred at the facility. LPA arrived at the facility at 9:25AM and met with David Barba, Assistant Principal . LPA singularly toured the facility. There were 10 children in care and 4 staff present upon arrival. The incident that occurred on 02/04/2025, was reported to the Department on 02/05/2025, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Child #1 and Child #2 engaged in an activity that was not appropriate. On 02/14/2025, LPA conducted an initial followed up on an incident that was reported to the Department on 01/30/2025, via email. LPA conducted interviews with Principal, Staff #1, Staff #2, Staff #3, Staff #4, and Parent #1 and Child #2. Child #1 was not present and had not been in care since 02/11/2025. During todays visit, LPA interviewed Child #1 and conducted a follow up interview with Staff #1. Staff #1 notified LPA that Child #1 returned to care on 02/28/2025. Based on information received from the interviews conducted, it was determined that there were no deficiencies found at this time. There were no deficiencies cited during today\342\200\231s inspection. The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted and report was reviewed with David Barba, Assistant Principal.
