What the inspector wrote

On 07/07/2021, Licensing Program Analyst (LPAs) Monique Ayala and Isabel Ortega met with the director, for the purposes of conducting a follow up complaint investigation for the above allegation. LPAs toured the facility and gathered the census of children present. There were 37 children in care with 7 staff. Based on LPA's observation the preponderance of evidence standard has been met, therefore the above allegation is found SUBSTANTIATED. The key pad for the main entrance is broken. A Type B deficiency will be cited under Title 22 Regulation Code 101238(a), see LIC9099D. An exit interview was conducted and a copy of this report was provided to the director along with appeal rights and Notice of Site Visit.
