What the inspector wrote

On 04/26/22 Licensing Program Analysts (LPA's) Justin Dorsey and Justeene Tamayo, met with Director Adrieenne Tolliver for a Case Management - Deficiencies inspection. Licensee Allea Adams later arrived to the facility to assist with the inspection. During the inspection the following was observed: LPA Dorsey and Tamayo observed Staff #1 in the facility's toddler program did not have a fingerprint clearance. LPA Dorsey and Tamayo observed Staff #2 supervising 25 napping children by themselves. Upon receipt of a Type A deficiency, a copy of the licensing report must also be posted for 30 days. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee must obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file. Copies of the reports must be provided to each parent when a Type A violation is cited along with Acknowledgment of Receipt of Licensing Reports LIC 9224. If these requirements are not met civil penalties per violation will be assessed. Facility cited Type A and Type B deficiency, according to California Code of Regulations Title 22. LIC809D issued for deficiency. An exit interview was conducted, a copy of this report was read and provided to Allea Adams.

    101230 Activities (c) A teacher-child ratio of one teacher supervising 24 napping children is permitted provided that the remaining teachers necessary to meet the overall ratio specified in Section 101216.3(a) are immediately available at the center. This requirement is not met as evidenced by: Based on observation Staff #2 was supervising 25 napping children alone, which poses a potential Health, Safety or Personal Rights risk to children in care.

    101212 (d)(1)(e) Upon the occurrence...a report shall be made to the Department by telephone or fax within the Department's next working day...a written report...shall be submitted within seven days following the occurrence of such event. Events reported shall include the following: Epidemic outbreaks. This requirement is not met as evidenced by. Based on observation, interview, and record review, Licensee did not notify the Department of the incident that occurred on 04/11/2022 within the time frame required. This is a type B deficiency...which can pose a potential Health and Safety risk to children in care.
