What the inspector wrote

On 11/6/2025, Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced case management inspection to follow up on an Unusual Incident, reported to the department on 10/22/2025. LPA toured the facility and took a census of how many children and staff are present today. LPA observed 14 children present with 4 staff. Members. LPA met with Denise Perez Program Management. According to the Telephone UIR RP, received notification from the school’s family advocate that there is a hand, foot, and mouth. The school was notified on 10/20/2025 that child 2 was diagnosed with hand, foot and mouth. The school was notified on 10/22/2025 that child 3 and child 4 was diagnosed with hand, foot, and mouth. A parent health notification was sent to all the parents. Staff sanitized all the toys and equipment indoors and outdoors. During the inspection, LPA toured the facility and obtained copies of the Hand, Foot and Mouth Disease (HFMD). LPA reviewed all four doctor’s notes. All children have returned to school. Parents were notified by phone, email, and Learning Genie. Parents were provided information. Handouts were given to the parents. Beautification Day was held at the facility. to clean and disinfect all materials indoors and outdoors. Maintenance cleaned the rugs. LPA concluded this case management inspection visit, No Title 22 violations have occurred, and no deficiencies cited. A notice of site visit was given and posted for 30 days. Exit interview conducted and report was with Denise Perez, Program Management.
