On 8/23/2024, Licensing Program Analyst (LPA) Loyce Phillips conducted an unannounced Case Management-Other Inspection. LPA met with Bianca Amezcua, Program Supervisor. No children were present during visit. The purpose of this visit is to obtain the newly hire Director's packet. LPA received and reviewed the following: 1. LIC 9096 (not completed). 2. Unofficial Transcripts 3. Mandated Reporter Training completed on 4/9/2024 4. CPR/First Aid completed on 4/10/2024 5. LIC 508 6. LIC 503 Health Screening Report with TB 7. LIC 501 Personnel Record 8. Immunization's with MMR, TDAP and TB LPA explained to the Program Supervisor additional information for Director's packet is required in order to qualified the Director. LPA reviewed Title 22 section 101215.1 Child Care Center Directors Qualifications and Duties with Program Supervision. An exit interview was conducted and a copy of this report was read and provided to Bianca Amezcua.
