What the inspector wrote

On Thursday, June 5, 2025, Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management inspection to follow up on a self reporting incident that occurred on April 29, 2025. LPA met with director Edith Murillo who guided LPA Rivera on a tour of the facility. LPA observed 15 children on the playground with 6 staff present providing care and supervision. LPA observed the facility to be within ratio. The incident a child slipped on the balancing equipment and fell landing on an outside table hitting forehead on the left side. LPA inspected the outdoor play yard and equipment. LPA observed the playground to have a soft padding, and the balance and furniture is age appropriate and the table is a medal with a tightly woven plastic. Based on the interviews and outdoor playground observation by LPA Rivera, the incident was an accident. Staff 1 told child not to be jumping on the balance beam and as staff 1 was walking towards the child, the jumped off and fell and hit head. No deficiencies given during this visit. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report was reviewed with director Edith Murillo.
