What Happened
Plan of Correction visit for deficiencies from 2/26/25 3-year inspection. Lead testing initiated with return visit scheduled for 4/8/25. TB clearance completed for all staff but immunization records, disaster plan, and teacher qualifications remain deficient.
Observations
- Lead testing in progress; inspector unable to complete sample collection on 3/25, returning 4/8/25
- Canopy ordered for playground shade as corrective action
- Only 2 of 8 staff have proof of immunizations (deficiency)
- Disaster and mass casualty plan not completed (deficiency)
- 4 of 7 teachers lack required units (deficiency)
- Toddler teacher missing infant/toddler specific units
- Assistant lacks required units
- Teachers enrolled in classes but not yet qualified

1.... a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by:Based on LPA observations and record review 6 out of 8 staff do not have proof of the required immunization's. This poses an potential health and safety risk to children in care.
