Incident investigation·September 30, 2025

On 9/30/2025, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced case management inspection due to an incident which occurred on 09/08/2025. The written report was mailed and received to the Regional Office on 9/11/2025. LPA arrived at the facility and met with Administrator Lindsey Convoy, who guided LPA on a tour of the facility. RM 1:\302\240 11 preschool children with a ratio of 2 teacher RM 2: 11 preschool children with a ratio of 2 teacher RM 3: 11 preschool children with a ratio of 2 teachers RM 4: 10 preschool children with a ratio of 2 teachers RM 5: 11 preschool children with a ratio of 2 teacher RM 6: 13 preschool children with a ratio of 2 teacher RM C202: 11 preschool children with a ratio of 2 teacher RM C203: 12 preschool children with a ratio of 2 teacher RM C204: 13 preschool children with a ratio of 3 teacher The incident which occurred, Child #1 tripped and scraped inside of mouth on hillside playground. Child #1 was present during the visit, LPA Morales observed Child #1 run outside the playground. Page 1 of 2 During staff interviews LPA was informed that Staff #2 and 3 had active supervision of the child who obtained an injury. Staff #3 informed LPA they saw it occur and were about to remind the Child #1 to stop and drink water in place, when Child #1 had the incident. LPA Morales inspected where the incident occurred. LPA Morales took photos of where the incident occurred. LPA Morales took photo of the water bottle, to verify if it was age appropriate. LPA Morales was informed by Staff #3, that Child #1 lost their footing. When asked if Child #1 was walking or running, Staff #2 stated Child #1 was "Walking, I mean barely they took a step." Staff #2 explained that they checked on Child #1, and noticed Child #1, was bleeding and looked to have a cut on their gum. Staff #2 informed LPA that Staff #2, helped rinse Child #1 mouth, was given an ice pack, then given a Popsicle to sooth their mouth inside. Per Staff #2, parents were notified of the incident. During Child #1 interview, Child #1 informed LPA they "fell down," while "drinking water." LPA Morales spoke with Parent #1 regarding the incident, Parent #1 has no concerns. LPA informed facility of the correct email address to submit the Unusual Incident Report LIC 624. During the inspection, LPA obtained a copy of the children's roster, obtained a copy of internal "Ouch Reports," conducted interviews with Staff, interviewed Child #1, interviewed Adult #1,\302\240took photos and made observations of where the incident occurred, collected text messages between facility and Parent #1. At this time, no further investigation is needed. There were no deficiencies observed in regards to todays visit. Notice of Site was given and must remain posted for 30 days.
