What the inspector wrote

LPA Cohen substantiated the allegation based on physical observation and interview with the director. The facility was cited a Type B violation. An exit interview and a copy of this report and Appeal Rights were provided to the director, Dejahnae Alexander. SUBSTANTIATED - A finding that a complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met.

    Outdoor Activity SpaceThe surface of the outdoor activity space shall be maintained: In a safe condition for the activities planned.This requirement is not met as evidenced of visual observation by LPA and interview with preschool director. LPA observed playground flooring tiles, in multiple areas, separated and not connected with each other creating a gap that presents a trip hazard for children in care when using the playground. This pose a potential risk to the health and safety of children in care.
