What the inspector wrote

Licensing Program Analyst (LPA) Nathanael Mooberry and Licensing Program Manager (LPM) Rita Ramos conducted an unannounced annual inspection to the above facility on 02/18/2026.\302\240 LPA and LPM arrived at the facility at 09:20 AM, identified self and met with Mr. Turner, EL Coordinator, who guided analyst on a tour of the facility. LPA provided EL Coordinator with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 1 classroom, Room #5. Facility operation hours are Monday to Friday from 08:00 AM to 02:30 PM, with the exception of Tuesday which is 8:00 AM to 01:30 PM.\302\240 This state preschool is located within the campus of an elementary school. All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff and children were present: Staff #1 and #2 with\302\24015 children. The facility was observed to be within the license capacity and limitations. This facility is funded by the California Department of Education and operating under Title 5 staffing ratios. Criminal record clearance was not reviewed. However, per condition of employment with the Los Angeles Unified School District, all individual\342\200\231s present have obtained a criminal record clearance or criminal record exemption. ---Page 1 of 4 The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated.\302\240 Children have their own cubby to store their belongings.\302\240 LPA did not observe children belongings touching. Mats and linens are provided for each child enrolled. Linens are washed weekly by school district.\302\240 LPA observed napping equipment stored in the classroom. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free. Age-appropriate sinks and toilets were inspected for availability and good repair in the restrooms. The classroom also has an additional sink for hand washing. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children.\302\240 A carbon monoxide detector was observed in the classroom and were operable. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free. Disaster drill log was available, last drill was conducted on 02/09/2026. Per EL Coordinator, there are no firearms stored on the premises. LPA discussed the safe sleep regulations with EL Coordinator and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed EL Coordinator of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children.\302\240 Plant Manager states that poisons are locked in the Janitor's closet. ---Page 2 of 4 All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin.\302\240 All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and are in good repair.\302\240 Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.\302\240\302\240 Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request.\302\240 The facility provides breakfast, lunch, and snack to children. Food is prepared in the main elementary cafeteria located at the facility. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.\302\240 All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time. LPA did not observe any pools or bodies of water at the facility. Facility has conducted water lead testing to comply with Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 requires all licensed Child Care Centers (CCC\342\200\231s) constructed before January 1,2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every five years after the date of the first test. Children\342\200\231s Records were reviewed. Inspection of required forms was made and documented on the LIC 857.\302\240 3 of 3 child files were complete. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed and 13 of 15 children present were signed in. LPA also reviewed staff records. The review of Staff records was documented on the LIC 859.\302\240 Staff present did not have proof of the AB 1207 Mandated Reporter Training certificate on file. LPA was povided copies of district mandated reporter training that does not meet the requirements. A Technical Violation was given.\302\240 Staff present did not have proof against measles, pertussis, and influenza on file. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. --Page 3 of 4 First Aid supplies were observed in the classroom. According to Nurse, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the nurse's office. Incidental Medical Services (IMS) policy was discussed.
