What the inspector wrote

On 8/12/25 Licensing Program Analyst (LPAs) Tyler Reyes conducted a Case management incident inspection to follow up on incident that was reported to the Department 7/28/25. LPA met with Site Supervisor Douglas who guided LPA on a tour of the facility. LPA observed (37) children in care. LPA observed (4) teachers and (3) aids. Hours of operation are Monday-Friday from 6:30AM-6PM. On 7/28/2025, an Unusual Incident Report (UIR) was submitted to the Department by Site Supervisor Douglas. The report stated that (3) children had confirmed cases of Hand, Foot, and Mouth Disease. Per Site Supervisor Douglas parents were notified on 7/28/25 by a letter. The letter informed families that there had been a reported case of Hand, Foot, and Mouth at the center. The letter provided information on the illness and initial symptoms. The letter advised parents to keep their child home if they became ill and to notify the facility. The letter encouraged parents to contact the facility with any questions. Based on an interview with the Site Supervisor Douglas the issue has been corrected. Per Site Supervisor Douglas the facility sanitized toys, blankets, and all indoor and outdoor equipment. Carpet cleaning and deep cleaning were completed over the weekend. The health department contacted and advised the facility to notify them if any additional cases occur. Per Site Supervisor Douglas the health department did not provide a case or report number and did not conduct further investigation. Per Site Supervisor Douglas no prior history of Hand, Foot, and Mouth. During the annual visit, LPA observed the facility to be clean. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days consecutive days . Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview conducted and a copy of the report and appeal rights were provided to the Site Supervisor Douglas. No deficiencies cited.
