What the inspector wrote

On 04/02/2026, Licensing Program Analyst Adrian Risher conducted a case management inspection regarding an incident that occurred on 03/11/2026 . LPA met with the Johana Partida , Director. LPA observed 27 children with 4 staff. On 03/11/2026, the facility reported an incident involving child 1 getting injured while playing on the play ground. On 04/02/2026, LPA conducted interviews with facility staff. LPA Risher received a copy of the doctor's note for child 1 dated 03/17/2026 from a follow up doctor's visit, accident reported dated 03/11/2026, written statements from staff, LIC 624 dated 03/11/2026, and facility roster. Staff reported child 1 was playing on the play structure on the play ground and fell down onto his right arm. There was 3 staff present on the play ground at the time of the incident. Staff rendered aid and notified the parents of child 1 at the time of the incident. Based on the investigation which included interviews with relevant parties and observations by the LPA, there was insufficient evidence to determine that any Lack of Supervision violations occurred. There were 3 staff present on the playground at the time of the inspection. No deficiencies will be cited today. An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Johana Partida, Director
