What the inspector wrote

On 07/11/2022 @ 11:48 AM, Licensing Program Analyst (LPA) Miriam Cohen conducted a Case Management Unusual Incident inspection to follow up on the self-reported incident that occurred on 07/06/2022 at Valor Christian Academy \342\200\223 Preschool located at 525 Earle Lane, Redondo Beach, CA 90278. The El Segundo Regional Office received the incident report on 04/08/2022. Upon arrival, LPA observed proper care and supervision. All center staff that was present during today\342\200\231s inspection had fingerprint cleared and associated to the designated license number. LPA interviewed the following: Witnessed teacher, Staff #1 (S1), assistant preschool director, and parent of child (Child #1 or C1) involved in the incident. Based on the information that were gathered through record reviews, video footage, and interviews, it revealed that there were five adults supervising 33 children in the preschool program on the day of the incident. Per assistant director, as documented in the UIR form LIC 624, S1 witnessed the incident and cared for C1. Three other staff members were also present but did not witness the incident. Based on the narrative, C1 was holding a water can, watering plants, swirling around in a circle, fell back, and hit the bike that was behind her. C1 hit the back of her head on the black top. Staff #1 rushed over, picked up and comforted C1. Ice packed was applied on head bump. The parent was notified immediately and gave consent for Staff #2 (S2) to ride with C1 in the ambulance on the way to the hospital. Parent met S2 and C1 in the hospital. C1 received an emergency care by medical provider. C1 was released and returned back to preschool on 07/08/2022 with a physician\342\200\231s note. At this time, based on the available information, it does not appear that this incident was the result of a Title 22 violation for lack of care and supervision. The content of this report was read and discussed in detail with Michette Sutherland, assistant preschool director. An exit interview was conducted; the notice of site visit must be posted for 30 days upon receipt.
