What the inspector wrote

On 11/3/25, Licensing Program Analyst (LPA), V. Wheatley met with Director Shanelle Muniz and conducted a case management inspection. During an investigation LPA Wheatley learned through interviews that the facility had six children with Hand, Foot and Mouth disease during August 18, 2025 and August 26, 2025. This must be reported to Community Care Licensing. The deficiency will be cited on LIC 809D. Exit interview was conducted. A copy of this report was read and provided to the director. The Notice of Site Visit will be posted for 30 days. Appeal rights provided.

In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.Title 17, Section 2500, lists the following reportable communicable diseases as of March 21, 1997:.....OUTBREAKS of ANY DISEASE (Including diseases not listed in Section 2500). This is evidence by: there were at least six children that had Hand, Foot and Mouth Disease. This is considered a communicable disease and were not reported to CCL. This is a potential risk to the health and safety of children in care.
