What Happened
Follow-up inspection for two incidents: lack of supervision on 11/19/24 where staff were unaware child was out of care, and child-on-child injury on 11/21/24. Staff failure to maintain supervision determined and Type B citation issued.
Observations
- Staff were not aware of child out of care during 11/19/24 incident (though child was within eyesight of other staff)
- Child-on-child injury incident occurred on 11/21/24 with no medical treatment sought
- Parents were notified of both incidents but no additional concerns expressed
