What Happened
Case management investigation of two personal rights violations reported on 3/19/2025 and 4/18/2025. Interviews with facility representative, staff, and children confirm allegations. Facility followed proper LAUSD protocols including reporting to LAPD. Two Type A citations issued.

(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1)No child(ren) shall be left without the supervision of a teacher at any time, .. This requirement is not met as evidenced by: Based on interviews, facility does have a plan of supervision but does need additional to meet the need for full supervision, which poses a potential health and safety risk or personal rights risk to children in care
