On 10/17/2025 at 12:45 PM Licensing Program Analyst (LPA) Claudia Kam conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 9/4/2025. LPA met with Facility Representative (FR) acting covering administrator, Principal Rhonda Granados. Census was taken. An incident was reported to The Department in regards to supervision. The facility reported this incident to the Department within the required 24 hours and submitted a written report via email within seven days, complying with Title 22 Regulation requirements. During this inspection LPA interviewed FR and staff involved, child continues to attend care. FR followed LAUSD protocols regarding these types of incidents, and reported to the Operations Coordinator. Facility Representative states that internal investigations are pending regarding the reported incident which includes interviews of parents and children and obtaining written statements from Staff and operations decision. FR states that children's Authorized Representatives were notified. LPA interviewed staff involved in the incidents and additional staff to confirm bathroom protocol. Interviews confirm the allegations reported for incident on 9/4/25. S2 and S5 confirmed that the child was left with S3 during the fire drill. S3 confirmed that she was asked to care for the child and that she accepted responsibility for the child. S4 confirms that she found the child alone in the bathroom with no staff supervising. Page 1 of 2 LPA staff interviews confirm that when possible, children are left with other staff regardless of the room they work in and returned to the their designated classroom. Staff interviews in addition confirmed that they are to check the bathroom before leaving for any other children. The facility is currently undergoing Child Care Licensing Department's TSP (Technical Support Program) training for supervision. Based on this information the following deficiencies on the LIC 809 D are being cited today 10/17/25. A notice of site visit was given and must remain posted for 30 days.
