What Happened
Case management visit for supervision incident reported 9/4/2025. Investigation confirmed child was left alone and unsupervised in bathroom during fire drill when assigned supervising staff did not follow handoff procedures. LPA determined Title 22 supervision violation. Facility undergoing technical support program training.

- Child left unsupervised in bathroom during fire drill
- Assigned supervising staff (S3) accepted responsibility but child was found alone
- Staff did not consistently follow bathroom check procedures before leaving

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidenced by: Based on interviews, it has been confirmed that a child was left alone without supervision during a fire drill with no other adults nearby which poses a potential health and safety risk or personal rights risk to children in care.
