What the inspector wrote

On September 11, 2025, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to address an injury that was observed during inspection. LPA toured the facility with the representative and observed ninety-five (95)-day care children and twenty-three (23) staff in active care. All adults 18 years and over have Criminal Record Clearance (DOJ) and Child Abuse Central Index Clearance and are associated with the facility. During inspection, LPA observed a child (C1) with a cast on their right leg almost to the knee. Representative stated C1 sustained the injury while playing outside on the playground at the day care. LPA conducted interviews with all relevant parties involved. LPA reviewed staff and child’s file and completed a safety inspection of the playground. Based on interviews and record review, during morning playtime, C1 was running on the grass with other children on the playground outside. C1 tripped while running, fell to the ground, and sprained their right ankle. C1 could not walk staff carried C1 to the bench and provided an ice pack. C1 was also carried by the staff to the front office. C1 went to the doctor and was provided with a soft cast to the right leg and has returned to the daycare. LPA did a safety inspection of the playground and observed the grass area where the injured occurred. The grass was worn away and there was more dirt in the area where the injured occurred. There were small indentations in the dirt, but the rest of the playground was observed safe. The staff notified C1’s parents but did not notify Community Care Licensing (CCL) by the next business day of the injury occurring. Although the staff took appropriate action in providing care, the reporting requirements violation did occur. There is one deficiency cited during this inspection. See the LIC 809D page attached to this report. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Rosemary Kober, Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
