What the inspector wrote

During the investigation LPA obtained a copy of the facility roster and conducted interviews with the Reporting Party, Licensee and Parent #1 through Parent #5. LPA obtained pertinent information regarding the allegation. During an interview with the Reporting Party, it was disclosed that the Licensee was out of the country from 03/04/2025 through 03/18/2025. During a file review, LPA was not able to find proof that the Licensee submitted a notification regarding their absence. The Licensee disclosed that prior to leaving out of the country, the Department was notified via email and via telephone. Licensee provided a copy of the email submitted to the Department as proof of notification. Upon reviewing the email submitted to the Department, it was observed thst Licensee submitted the notification to an LPA in a different regional office. LPA conducted an initial 10 day visit to the facility on 04/16/2025. At no time did the LPA observe the facility to be out of ratio. LPA observed all individuals present to be adults and listed on the Guardian Facility Roster. While touring the facility, Licensee disclosed that there are pets in the home. LPA observed the yard accessible to children to be separate from the yard Licensee pets are kept. The yards are separated by a wooden fence that is kept closed with a latch that is inaccessible to children in care. At no time did the LPA observe any animal feces in the play area accessible to children. LPA also inspected the side of the home where children enter during drop off and pick up, and no animal feces was observed. LPA conducted subsequent visits on 05/14/2025 and 06/12/2025. At no time did the LPA observe the facility to be out of ratio. LPA observed all individuals present to be adults and listed on the Guardian Facility Roster. While touring the facility, LPA did not observe any animal feces in the areas accessible to children. During todays visit, at no time did the LPA observe the facility to be out of ratio. During interviews with parents, information regarding Licensee being out of the country were confirmed. Disclosures were made that Licensee notified parents prior to Licensee's absence. There were no disclosures made to support the other allegations . ---Page 2 of 3 Based on the investigation conducted, it has been determined that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. A copy of this report, appeal rights, and Notice of Site Visit was provided.
