What the inspector wrote

On 02/04/2025 Licensing Program Analyst (LPA) Ranita Richmond arrived at the above named home to conduct a Plan of Correction visit and was met by Licensee Cecilia Ponce. LPA observed 7 children in care being supervised and cared for by licensee and 1 fingerprint cleared assistant. On 11/26/2024, Licensee was cited for the following: 1. Licensee will ensure that open face heaters shall be screened to prevent access by children. 2. Licensee will ensure that Bumper pads shall not be used in cribs. 3. Licensee will ensure that all detergents, cleaning compounds, etc. are inaccessible to children in care. 4. The provider shall check and document the all infants sleep, using an infant sleep log for all infants up to 24 months of age. 5. Licensee will ensure that personnel files contain a signed and dated copy of the Notice of Employee Rights LIC 9052. 6. Licensee will ensure all employees of licensed child day care facility shall complete the mandated reporter training every two years as necessary. 7. Licensee will ensure that children's file contain a signed affidavit stating that the parent has been informed that the family child care home does not carry liability insurance , LIC 282. During visit LPA Richmond observed the following: 1. LPA observed printed copies of mandated reporter completion certificates for licensee and 1 fingerprint cleared assistant. 2. LPA observed cribs free of bumper pads. 2 of 7 Citations issued on 11/26/2024 has been cleared. An exit interview was conducted. A copy of this report, notice of site visit, deficiencies clearance letters were discussed and provided to Licensee Liliana Ponce.

LPA observed screen propped in front of heater.

LPA observed children's files missing LIC 282.

LPA observed cribs with loos articles and objects.

(3) There shall be no objects hanging above or attached to the side of the crib.This requirement is not met as evidenced by: Based on observation the licensee did not comply with the section cited above in 1 out of 1 which poses/posed a potential health, safety or personal rights risk to persons in care. LPA observed toys attached to side of crib.

    1597.622 Employees or volunteers at family day care home; immunization requirements; records; exemptions(c) The family day care home shall maintain documentation of the required immunizations...This requirement is not met as evidenced by: Based on record review the licensee did not comply with the section cited above in 1 out of 1 which poses/posed a potential health, safety or personal rights risk to persons in care. LPA reviewed staff files and observed missing TB.

LPA observed cleaning compounds, detergents, and more in laundry room and bathroom accessible to children in care.
