What the inspector wrote

On 02/04/2025 Licensing Program Analyst (LPA) Ranita Richmond arrived at the above named home for an unannounced Case Management Deficiencies visit and was met by Licensee Liliana Ponce. LPA observed 7 children in care being supervised and cared for by licensee and 1 fingerprint cleared assistant. LPA Richmond toured the home inside and outside for a Health and Safety inspection. During walk through LPA observed an open-faced heater in the living room that has a screen that is propped against the heater, but not installed to prevent access by children in care. Repeat citation issued. See LIC 809D. See LIC 421FC. LPA observed detergents, cleaning compounds, medication, etc. were readily available and stored where accessible to children in care in the dining room, laundry room, and bathroom. Repeat citation issued. See LIC 809D See LIC 421FC.. LPA observed cribs with items connected to the side of the crib. Citation issued. See LIC 809D. Licensee observed one infant sleeping on stomach inside of crib with loose items inside. Citation issued. See LIC 809D. LPA observed that licensee does not have present for viewing infant sleep log. Repeat citation issued. See LIC 809D. See LIC 421FC LPA reviewed 7 children’s files and observed them to be missing affidavits of liability insurance. Repeat citations issued. See LIC 809D. See LIC 421 FC. LPA reviewed file of sleeping infant and observed section D of LIC 9227 incomplete by provider. Infant should be placed on back for sleeping and remain on back for sleeping. Citation issued. See LIC 809D. LPA reviewed employees files and observed employees rights (LIC 9052) missing for assistant. Repeat citation issued. See LIC 809D. LPA observed TB clearance missing for assistant. Citation issued. See LIC 809D. An exit interview was conducted, a copy of this report and appeal rights was read and provided to Licensee Liliana Ponce. Notice of Site Visit was provided and required to be posted for 30 days.

LPA observed screen propped in front of heater.

LPA observed cribs with loos articles and objects.

LPA observed toys attached to side of crib.

LPA reviewed staff files and observed missing TB.
