What the inspector wrote

On April 30th, 2026, at approximately 9:15 AM, Licensing Program Manager (LPM) Emiko Bell and Licensing Program Analyst (LPA) Janet Gil conducted an unannounced plan of correction (POC) visit. LPA met with the Director, Anna Shirokova, and explained the purpose of the visit. Present during today's visit included 10 staff, including the director, and 40 enrolled preschoolers. On April 8, 2026, LPA Gil conducted an unannounced annual inspection. During the inspection, the licensee was issued three Type B citations for the following: missing Mandated Reporter Training for six staff, no carbon monoxide detector in the facility, and accessible thorns on rose bushes in the outdoor entrance area. A Plan of Correction was discussed with the licensee. The Director requested additional time to correct the accessible rose bush deficiency. LPA will follow up with the Director to verify correction of the physical plant deficiency by 05/08/2026. During today's visit, LPM/LPA observed an electric carbon monoxide detector installed in the kitchen area of the preschool in Classroom 1. The carbon monoxide detector was tested and is operable. The files of the six staff were reviewed and five of the six staff files now contain the certificates of completion for the Mandated Reporter training (see 812 for documentation of the sixth employee). During the visit, LPA observed the Notice of Site Visits for the 04/08 and 04/29 visits to be properly posted and available for review by the entrance of the facility. Two deficiencies cited on April 8th, 2026, have been cleared during today's visit. LPA provided the licensee with two Letters of Deficiencies Cleared on this date . No deficiencies were cited today. A Notice of Site Visit was given and must remain posted for 30 days.Exit interview conducted and report was reviewed with Director Anna Shirokova.
