What the inspector wrote

Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced annual random site inspection. LPA met with Center Director, Guadalupe Grijalva. LPA inspected the inside and outside of the facility. The facility is located on the property of the El Segundo USD. Five rooms inspected & 2 additional rooms used for special services. School hours are 8:00am to 4:00pm Monday through Friday during regular school hours. At the time of the inspection the Summer Program was in session Monday- Friday from 9:00a.m. to1:00p.m Upon arrival LPA observed a total of 34 children being supervised by 4 teachers and 6teachers aides. LPA observed children during activity times. Preschool utilizes classrooms #1through classroom #7 for instruction Two additional classrooms (Classroom #1 & Classroom #2) are used special services (Movement, Speech, Sensory & Parent Interviews). Classroom #5 and # 7 had a total of seventeen children in the classroom. Furniture and equipment were inspected for age appropriateness and good repair in each classroom. Telephone service, heating, lighting and ventilation were evaluated. LPA observed individual cubbies for children's belongings outside each classroom. The facility uses the director\342\200\231s office as an isolation area when children are sick. LPA observed a cot for children that are sick. Age appropriate sinks and toilets were inspected for availability and good repair. LPA observed restrooms for boys and girls. Toilet paper, paper towels were observed inside the restroom. First Aid supplies are kept in classroom. LPA observed fire extinguishers in each classroom and sign in/sign sheets were observed outside of each classroom. Fire and disaster drills are conducted monthly. The center does not provide snacks and lunches. Parent\342\200\231s provide lunch daily for their kids. Water is available inside and outside of the classrooms. All classrooms had a carbon monoxide. The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days . If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224). An appeal may be filed, in writing 15 business days from the date of receiving the penalty assessment. As of September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit. The licensee was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State\342\200\231s licensing role, provide information to the public and parents on childcare licensing, and provide many other helpful resources to the licensees and the public. Out of the files only three teaching staff will need the mandated reporter training to renew it. All Teaching staff were current with the First Aide& CPR. For the immunization records that were reviewed only two teachers will need to turn in their immunization record. LPAspoke to the Director to send in documentation. LPA reviewed a total of 10 children c files which were complete There were no bodies of water observed in the playground area. The outdoor playground was inspected and was observed to be free of hazards, loose and sharp parts. LPA did observe enough shade. The playground was observed to be properly gated all around. Equipment was inspected for safety, cushioning material, good repair and age appropriateness. LPA observed a gated patio for sitting for children. LPA observed play equipment securely anchored for safety. The following forms were observed to be posted. The facility license, Parent's Rights Poster (PUD 393), Personal Rights (LIC 613A), Emergency Disaster Plan (LIC 610). All medication is stored in lock box in each classroom. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Per the Title 22 regulations, on 5/19/2017, the above facility will not receive Type B/Type A citations. An exit Interview was conducted, a copy of this Report, and a Notice of Site visit was provided. The following was discussed with the licensee: Mandatory Forms for the children\342\200\231s files and provider\342\200\231s files, Requirements for fire drills, earthquake drills and documentation for both. Role and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care . Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov . The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility\342\200\231s phone number at all times ; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises. State law prohibits baby walkers, bouncy seats, exer-saucers and any other items that fall into that category. All infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome), and that the Provider is required to wash hands after every diaper change and to never shake a baby to prevent the Shaken Baby Syndrome . Only children eating may be in high chairs and that car seats are utilized only for transportation. The ADA requires that child care providers not discriminate against persons with disabilities on the basis of disability, that is, that they provide children and parents with disabilities with an equal opportunity to participate in the child care center's programs and services. Requirements for fingerprint clearances and associations were discussed with the licensee. Licensee can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1 st offense and up to $3000.00 for the 2 nd offense within a 12 month period. The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation. The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents.
