On 08/19/2025 at 2pm, Licensing Program Analyst (LPA) Devon Carus conducted an unannounced Plan of Correction inspection to determine if the deficiencies cited on 8/7/2025 have been cleared. LPA met with Sun Ko, Director and explained the purpose of the visit. LPA observed 12 children in care being supervised by 3 staff. On 8/7/2025 the facility was cited for 3 loose water hoses, and two broken toy cars in the outdoor play yard while children were present resulting in a Type B Violation. On 8/19/2025, LPA Carus toured the indoor and outdoor play areas of the pre school and observed the hoses to be removed, as well as the toy cars. Director also sent photos to the email of LPA Carus confirming the necessary corrections. On 8/7/2025 the facility was cited for 3 expired food products in the designated Preschool refrigerator, resulting in a Type B Violation. LPA Carus instructed director to dispose of these food items immediately. On 8/19/2025, LPA Carus toured the kitchen area of the pre school and observed no expired food, as well as written dates on prepared food. Additionally, director has created and submitted the written plan for maintaining proper food storage to the email of LPA Carus confirming the necessary corrections. On 8/7/2025 the facility was cited for care and supervision regarding staff being outside of visual range of the children resulting in a Type A violation. On 8/19/2025, LPA Carus toured the school and observed all staff within visual range of each enrolled child. Director also instructed staff to watch the assigned video titled Supervising Children in Child Care Centers and has submitted each staff member\342\200\231s responses to the email of LPA Carus. Additionally, director is providing the report regarding this Type A violation to all enrolled children\342\200\231s parents and is collecting the corresponding signed LIC 9224 forms to add to the children\342\200\231s files. Additionally, director confirmed that she will be providing the report indicating the Type A violation as well as LIC 9224 form to all parents who enroll in the upcoming 12 months. On this date, the necessary corrections to clear this violation's POC have been met. Based on observations made by the LPA, no deficiencies will be cited today . Exit interview was completed with Director, Sun Ko. A notice of site visit was provided, and must remain posted for 30 days.
