What the inspector wrote

On 03/05/2024 Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced Case Management - Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 02/29/2024. Upon arrival, LPA met with Karen Cohen, Field Director, and explained the purpose of the visit. LPA toured the facility both inside and outside. LPA observed 66 children in care and 11 staff members during today’s inspection. According to the UIR, on 02/27/2024, the child named in the report did not tell the teacher that she was hurt, but when the dad came to pick her up, the child told Dad she fell outside and hurt her arm. When they got home the child said she fell out of her cot at school. Dad informed the center via email that his child had fracture the growth plate in her wrist. During today’s inspection, LPA conducted staff interviews and a child interview. Additionally, LPA obtained the Children’s Roster, Personnel Report (LIC500), Personnel files, Parent Handbook, Preschool’s Daily Schedule, and time sheets. Based on the information gathered today, further investigation is needed. An exit interview was conducted and a copy of this report (LIC 809) and Notice of Site Visit were provided to Field Director, Karen Cohen.
