On 06/13/2025 Licensing Program Analyst (LPA) Cristina Castellanos conducted a Case Management Incident inspection to follow up on the self-reported incident that occurred on 05/27/2025 at the above-mentioned facility. The El Segundo Regional Office received the incident report on 06/03/2025. LPA Castellanos met with Director, Teresa Antonelli and stated the purpose for the visit. Upon arrival, LPA observed 95 children being supervised by 15 adults. All center staff members present during today’s inspection had fingerprint clearance and associated to the designated license number. On 06/03/2025, officer of the day Licensed Program Analyst (LPA) R. Richmond received a call from Teresa Antonelli, Site Director in connection to an Unusual Incident that occurred at the facility in regard to a child being left unattended in Classroom D2 for two minutes. Through documentation, interviews, and a physical plant inspection of the site of the incident, it was revealed that at the time of the incident during a transition from the outdoor play area into the classroom’s restroom staff member 1 (S1) left a child 1 (C1) unattended in the D2 classroom from 6:20pm to 6:22pm. Per the Director, C1’s parents were notified of the incident. Based on observation, record review and interviews of all relevant parties the facility did not comply with requirements in section 101229(a)(1) Responsibility for Providing Care and Supervision the licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. It was found that staff 1(S1) left a child 1 (C1) unattended for 2 minutes, which poses an immediate risk to the health and safety of the children in care. Continue There was a Type A deficiency cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1. See LIC 9099-D for additional information. Additionally, a Civil Penalty (LIC421 Repeat Violation) was access as well. Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a Type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgment of Receipt (LIC9224) form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit. An exit interview was conducted, and Plan of Correction was reviewed and developed with the Director.
