What the inspector wrote

On 04/14/2026 , Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced case management incident inspection to follow up on a self-reported incident that occurred on 04/10/2026 at the above referenced facility. LPA met with Director Teresa Antonelli and disclosed the purpose of the visit. LPA observed eighty (80) children in care and fifteen (15) staff members providing supervision. On the evening of 04/10/2026, Director Antonelli notified LPA Castellanos via email that she had attempted to contact the department to report an incident that occurred that morning but was informed by a staff member that the office was closed and to call back Monday morning. On 04/13/2026, Officer of the Day LPA D. Whitmore received a call from Director Antonelli regarding the Unusual Incident involving child (C1). According to the report, at approximately 12:05 p.m. during the transition from toileting to rest time, C1 stepped onto her cot, slipped, and struck the left side of her head on a table. The impact caused a cut behind C1’s left ear and blood was observed inside the ear canal. Director Antonelli stated that first aid was administered and parents were contacted immediately. Parent took C1 to Urgent Care, where C1 was medically evaluated, cleared to go home, and instructed to monitor for signs of concussion for the next 24 hours. C1 returned to school on Monday, April 13, 2026, without a doctor’s note. During today’s inspection, LPA interviewed C1 and relevant staff members. LPA also received the LIC 624 Unusual Incident Report for the incident dated 04/10/2026 from Director Antonelli. Based on the information obtained, there were no citations issued during today’s inspection. An exit interview was conducted. A copy of this report (LIC 809) and the Notice of Site Visit were provided to Director Teresa Antonelli.
