What the inspector wrote

On 9/25/24 at 12:30pm Licensing Program Analysts (LPA\342\200\231s), Jeanine Lipsey and Dawn Dowling conducted an unannounced Required Annual Inspection and was met by Licensee Margarit Warzhapetian. Entrance checklist was provided. LPA observed 5 children in care today. Days and hours of operation are Monday through Friday 8am to 6pm. Capacity as specified on the license is being maintained. The licensee Mom and sister was also present. Signed Affidavit Regarding Liability Insurance Is in the child files. LPAs observed the following required postings: License, LIC610A Emergency Disaster Plan, PUB 394 Notification of Parents\342\200\231 Rights Poster, and LIC 9148 Earthquake Preparedness Checklist. LPAs advised all LIC9213 Notice of Site Visits shall be posted for 30 days after each site visit. LPAs advised, any licensing report documenting a Type A citation must be posted for 30 days. LPAs advised a disaster drill log shall be posted with disaster drills completed at lease every six months. LPAs toured the home inside and outside. The licensee's home is a single story, 3 bedrooms, 2.5 bathrooms home with a living room, family room, kitchen, laundry room, and attached garage made into a bedroom. Main care is provided in the living room, family room, 2 bedrooms and the backyard for outdoor activities. Off limit areas include master bedroom, laundry room, master bathroom, and the garage. LPAs did not observe any poisons resent during the inspection. Detergents, cleaning compounds, medication and other hazardous item were not made inaccessible to children. Per applicant, there are no firearms or weapons. There is a pool in the backyard. LPA observed the pool fencing (mesh fence) to be 5 feet high. Both gates to the pool were tested and observed to be self-closing, self-latching and swings away from the pool area. LPAs observed the home to be free from defects or conditions which may endanger the children. Safety gates and locks were not installed for the off-limit areas. Safety locks for the doors need to be installed to render the rooms inaccessible to the children. page 1 of 4 LPAs observed fully stocked first aid kit. LPAs observed a working carbon monoxide detector in the home, LPAs was able to hear a successful test. Smoke detectors are located in the hallway, the living and family room. Fire extinguisher 2A10BC or larger was observed, located in the kitchen and was serviced on 3/18/24. Electrical outlets were observed to be inaccessible to children in care with the use of plug covers. The home was observed to be clean and orderly with heating and ventilation. LPAs observed safe toys, play equipment, and materials. LPAs verified the home has a working telephone service. There are no stairs in the home. LPAs observed the outdoor area to be fenced and free from hazards LPA observed multiple play equipment in good repair. Licensee ensures that children in care are always supervised. Licensee\342\200\231s pediatric CPR/First Aid was completed on 8/17/24. Mandated Reporter Training was not completed. The licensee owns the home. On this date, 09/25/24, the California Attorney General - Megan's Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. For additional information regarding the inspection and its tools and methods, please visit the Program website at: www.cdss.ca.gov/inforesources/community-care-licensing/inspection-process . page 2 of 4 Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Childcare Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPAs discussed the safe sleep regulations with licensee and discussed the Childcare Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and\302\255resources/ safe sleep as an additional resource. LPAs also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Margarit Warzhapetian confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Page 3 of 4 Based on LPA's observations, the following deficiencies listed on the attached LIC809D (deficiency page) are being cited in accordance with Title 22, Division 12, Chapter 3, of the California Code of Regulations. Deficiencies that are being cited need to be cleared to protect the children's health & safety. The following corrections proof are due to LPA by 10/9/24 via email. 1. The pool gates need repair. 2. The 4 cabinets in the kitchen need locks. 3. 4 Safety locks for the doors need to be installed to render the rooms inaccessible to the children. 4 15-minute charts need to be completed 5. The mandated reporter training needs to be completed. 6. Fire drills need to continue. 7. Safety locks for the doors need to be installed to render the rooms inaccessible to the children. Exit interview conducted and report was reviewed with Licensee Margarit Warzhapetian. A notice of site visit was given and advised it must remain posted for 30 days.
