What the inspector wrote

On 08/07/2025, Licensing Program Analyst (LPA) Tyra Chavies conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 07/03/2025. Upon arrival, LPA met with licensee, Maria Banes.  LPA informed director about the purpose of the visit and toured the facility. LPA observed 22 children being supervised by 5 teacher. Incident Details: 7/7/025 Licensee, Maria Banes, witnessed child 1 fall from a scooter while playing in the backyard. RP states the child fell and landing on their bottom. On 08/07/2025, LPA Chavies observed children in care, conducted interview with director and P1, and received facility roster. Based on interviews conducted the facility followed proper procedures and did not violate the children's personal rights. No deficiencies were cited. Exit interview was conducted and a copy of the report was provided to licensee, Maira Banes. Notice of site visit was given and must remain posted for 30 days.
